Direct Primary Care Disclaimer

Direct Primary Care (DPC) at Next Level Healthcare is available only to patients who do not have health insurance or have a health insurance plan that we are not in-network with.

Our DPC model is designed to provide transparent, affordable, and patient-centered care without the restrictions of traditional insurance billing. We do not bill insurance for services included in the DPC membership. Patients are encouraged to maintain a separate insurance plan for services not covered under DPC, such as hospitalizations, specialist visits, and emergency care.

18-64 years old

65 years old and older

$75 MONTHLY FEE

Your flat monthly membership fee covers all your office visits, virtual visits and follow-up communication. It also gives you direct access to your provider via secure call/text/email.
Additionally, membership includes one (1) comprehensive routine exam annually, which includes the following labs at no cost: CBC, CMP, TSH, Lipid panel, Hemoglobin A1C, and Urine Analysis.
In addition to the monthly membership fee, there is a one time enrollment fee of $100.
No long-term commitment is required. We request a 30-day notice for membership cancelations.

65 years old and older

$95 MONTHLY FEE

Your flat monthly membership fee covers all your office visits, virtual visits and follow-up communication. It also gives you direct access to your provider via secure call/text/email.
Additionally, membership includes one (1) comprehensive routine exam annually, which includes the following labs at no cost: CBC, CMP, TSH, Lipid panel, Hemoglobin A1C, and Urine Analysis.
